Registration

JKC will be accepting packets for the 2024-2025 school year.

The following information will be required for registration:

1. Copy of birth certificate/proof of birth
2. Copy of Social Security card
3. Shot record with up-to-date shots / or appointment card
4. Physical within the past year or appointment card
5. Insurance card
6. Proof of address (Recent utility bill or lease) 

In order for the registration to be considered complete, ALL of this information MUST be turned in with the registration packet.
